Diagonosing Problems with The Sims 2 & Custom Content:
Furniture, Walls & Floors work but there are no custom clothing, make-up or genetics in my game even though I installed them the same way?
Skins items are not always read by the game when placed in the Downloads folder, additionally for users without Nightlife or any of the other later Full Expansions, putting skins items in sub-folders will also cause the game to ignore them.

1. Move all Skins texture and mesh files out of your Downloads folder and place them in your Saved Sims folder:- 

Avoid putting them in sub-folders within the Saved Sims folder unless you have at least one of the later Expansion packs (not Stuff packs)
